Why microneedling and exosomes are almost always sold together

It is a fair question whether pairing them is clinical sense or a neat upsell. The answer is genuinely the former, and the reason is molecular size.

Microneedling is offered almost everywhere, usually with something applied afterwards — exosomes, PRP, a growth factor serum, a vitamin cocktail. It is reasonable to wonder whether that pairing is clinical logic or a convenient way to increase the price.

In this case the logic holds, and the reason is simple physics.

What microneedling does on its own

A device passes fine needles into the skin at a controlled depth, creating thousands of micro-channels. Each is a tiny, precisely controlled injury. The skin's response to controlled injury is to repair — laying down new collagen and elastin in the process.

This alone is an effective treatment, well evidenced for texture, fine lines, pore appearance and shallow scarring. Marketed as collagen induction therapy, and the name is accurate.

Why the channels change everything

Intact skin is an extremely effective barrier. It has to be. The consequence is that most large molecules applied topically simply do not get in — they sit on the surface until they are washed off.

Exosomes are large. Applied to intact skin, effectively none of the active fraction reaches the depth where it could influence anything. Applied immediately after microneedling, while thousands of channels are open, a meaningful proportion does.

So is it worth the extra?

The honest answer: microneedling alone is a good treatment, and adding a delivery-dependent active makes clinical sense while the channels are open. Whether exosomes specifically justify the £240 difference over a course of PRP is a judgement call — the evidence for exosomes is younger.

What is not a judgement call: if you are having microneedling, having something delivered during the window is better use of the appointment than having nothing.

Depth matters more than the add-on

The variable that most affects your result is the depth the needles are set to, and that should change across your face within a single treatment.

A practitioner running one depth across the whole face is either being too cautious to help the cheeks or too aggressive around the eyes. Ask what depth they are using and why. It is a fair question and a revealing one.

Downtime

Proportional to depth. A superficial glow treatment leaves you pink for an afternoon. A proper collagen-induction depth leaves you visibly red for 24 hours, pink for another day, and flaking lightly by day three or four.

If you were told to expect no downtime from a microneedling treatment, it was almost certainly done too superficially to change anything structural.

Course and timing

Three sessions, three to four weeks apart — enough time for one repair cycle to complete before the next stimulus. Collagen remodelling continues for three to six months after the last session, so the honest assessment point is twelve weeks after finishing, not immediately.

For acne scarring, plan on more than three and a longer horizon.

Why are exosomes applied after microneedling?

Exosomes are large molecules that cannot penetrate intact skin. Microneedling creates thousands of micro-channels that let a meaningful proportion reach useful depth. The channels close within a few hours, so the product must be applied during and immediately after treatment.

Is microneedling alone effective?

Yes — it is well evidenced for texture, fine lines, pore appearance and shallow scarring on its own. Adding a delivery-dependent active while the channels are open makes better use of the same appointment.

How deep should microneedling be?

It should vary across the face: around 0.5 mm on the forehead and around the eyes, 1.0 to 1.5 mm on the cheeks, and up to 2.0 mm on scarred areas. A single depth everywhere is a warning sign.

How much downtime does microneedling have?

Proportional to depth. A proper collagen-induction treatment leaves visible redness for 24 hours, pinkness for another day, and light flaking by day three or four. No downtime usually means it was too superficial to change anything.
